Alternatively, registration forms can be collected at the reception.
Following registration, you may be required to attend a new patient health check with the Nurse depending upon your co-morbidities as it can take 8 - 12 weeks for your medical records to arrive at the practice. Please bring any medication you are currently taking for it to be added to our system.
Medical treatment is available from the date of registration. Please contact reception for further information.
